FAX - Submit your completed order form via fax to 1.888.294.6376.
PHONE - Call in your order by phone at 1.800.423.0071.
MAIL - Send your completed order form to PO Box 365, New Windsor, MD 21776.
E-MAIL - Send the item numbers and quantities along with your customer number to orders@serrv.org.
